The who/what/when/where/how of filling out and turning in Eligibility Forms
- WHO – ALL 4-H Members, Public School, Private School, and Homeschool, are required to submit an Eligibility Form for every Event/Competition.
- WHAT – A 4-H Eligibility Form serves two purposes:
- Inform the school if your 4-H Member will be absent for a 4-H related event on the days listed on the form, to avoid an unexcused absence.
- Ensure that your 4-H Member is passing in accordance with the requirements of the Texas Education Code
- WHEN (you need one) – Eligibility Forms are required whenever you participate in any competitive 4-H event or miss school for a 4-H activity. This includes major stock shows, county fairs, roundups, judging contests, 4-H presentations, etc. When in doubt, fill it out!
- WHEN (to turn one in) – You may turn in an Eligibility Form as soon as you know your 4-H Member will be participating in an Event, but it MUST be turned in at least two (2) weeks prior to the contest/show.
- WHERE – Turn in all Eligibility Forms to the Extension Office. The Extension Office will send it to the school at the appropriate time for approval. If approval is not granted, the parent/guardian will be contacted as soon as possible.
- Once you complete the form (see “How”) and save it, there are three ways to send it to the Milam County Extension Office:
- Email it, straight from your device or computer, to kimberly.mcelwrath@ag.tamu.edu at the Milam County Extension Office. This is the quickest and easiest way.
- Print and Mail it to the County Extension Office at the address below. Please note that the County Extension Office must RECEIVE the Form two (2) weeks prior to the event.
- Print and Hand deliver to the County Extension Office during office hours at the address below, or for after hours we have a drop box, marked “4-H / Texas A&M AgriLife” located on the outside of the building, for your convenience.
- Pictures of Eligibility Forms will no longer be accepted. We have had too many that are unreadable when printed and/or scanned.

- HOW – The link at the top of this page is the EASY-FILL FORM you can complete on your mobile device or computer, and you can send it straight from there; no need to print it unless you want a paper copy for your records.

- You will need to save/download the form to your device/computer before emailing or it will send a blank form.
